  • The SF-36® Health Survey is a questionnaire with 36 standardized questions (items) used to assess generic health outcomes, which are often referred to as patient-reported outcomes (PRO) or health-related quality of life (HRQOL). The SF-36 measures eight domains of health outcomes: physical functioning, role-physical, bodily pain, general health, vitality, social functioning, role-emotional, and mental health.
